Environmental hazards and incidents are reported
All hazards and incidents shall be reported immediately to the supervisor. A hazards or incidents report shall be completed regardless of the severity of the hazards or incidents. Management shall review all hazards or incidents to determine the root cause and commit resources as follows:
• Training
• Servicing of plant or equipment
• Review of procedures
• Supply of additional PPE
• Supply of additional or appropriate equipment
• Review of the Management System
Management shall decide the appropriate action to prevent the re-occurrence of the hazards or incidents. Where an incident is reoccurring, work on that task must cease, until a thorough investigation identifies the root cause, and the CEO has approved return to work.
