All evidentiary criteria must be met in order to be considered for review by an authorized auditor.Audit Evidentiary Criteria: Evidence Required to Address CriteriaCheck off each criteria with satisfactory evidence. Company Manual Sign off pages: Policy & Procedures manual, Emergency Procedures Manual, New Worker Induction Booklet, Contractors Agreement, WHSMP / SWMS documents Meetings: Attendance sheets relating to each meeting in the attached getting started email meetings forms Inspections: Inspection checklists relating to each inspection in the attached getting started email inspection forms Staff List (current): Provide a copy of your current payroll staff / worker list or a screen shot of the Human Resources, People screen Training / Qualifications: Undertake a Training Needs Analysis and provide current training qualifications or certificates reflecting the person’s relevant training for their role/duties (e.g. White card, Class B / A etc) for EACH worker Licences: Provide a copy of your company’s current licences or registration (ASIC). Also licences for any Supervisors. Insurances: Provide copies of your company’s current insurance policies (e.g. Workcover, Public Liability, Professional indemnity, Contractor all risk, as appropriate) Contractor Agreements: Scan in any signed Contractor Agreements that you may have. Have you satisfied yourself that your Contractors meet the same criteria that you are attempting to gain? Legally they should meet the same benchmarks as you as they are also a PCBU Incidents: You need to scan in any incident reports that you have over the last 2-month period. It is also advisable that if you have had recorded incidents that you also provide notes on any corrective actions taken to address the failures that lead to the incident including meetings, policy reviews etc Safety : Click the View Hazard register button on the organization screen and it will self-populate under attachments!
