Public health and safety hazards and incidents are reported

All hazards shall be reported immediately to the supervisor. A hazard report shall be completed regardless of the severity of the situation. Management shall review all hazards to determine the root cause and commit resources as follows:

• Training
• Servicing of plant or equipment
• Review of procedures
• Supply of additional PPE
• Supply of additional or appropriate equipment
• Review of the Management System

Management shall decide the appropriate action to prevent the re-occurrence of the hazards. Where a hazard is re-occurring, work on that task must cease, until a thorough investigation identifies the root cause, and the CEO has approved return to work.
